Module 3
Work through each step below, then combine your answers into a single AIM statement sentence. Don’t worry about getting it perfect on the first try—AIM statements are living documents that should be refined as you learn more about your data and system.
- Identify your focus population: e.g., adolescents in the ED, students referred by school counselors, children in your primary care panel
- Define a measurable outcome: e.g., screening completion rate, time to appointment, follow-up completion rate
- Establish your baseline: What does the data show now? Even an estimate is a useful starting point that can be refined over time
- Set a realistic but ambitious target: What does success look like?
- Choose a timeline: What is a reasonable timeframe given your team’s capacity?
