Project Overview

Goal 1: Enhance statewide emergency department/EMS pediatric CQI capabilities. Objectives: (1) ensure that each of the eleven EMS regions have a formalized CQI subcommittee of their EMS Regional Advisory Board; (2) facilitate conduction of one CQI activity in each EMS region annually; (3) develop an EMSC quality improvement educational model.
Goal 2: Conduct a pediatric interfacility transfer quality improvement monitoring activity in two EMS regions within the state. Objectives: (1) develop and pilot a project monitor tool; (2) collect and analyze project data and develop reports; (3) develop an interfacility transfer educational workshop and transfer resource document; (4) increase to 50% the number of hospitals participating in the facility recognition process that have interfacility transfer agreements for critically ill and injured pediatric patients

Deficiencies in these areas have resulted in historically poorer outcomes for pediatric patients versus adults. Since 1985, federally funded Emergency Medical Services for Children (EMSC) programs in each state have been working to improve the quality of pediatric emergency care. The Health Resources and Services Administration now requires that all EMSC grantees report on specific performance measures. This includes implementation of a standardized system recognizing hospitals that are able to stabilize or manage pediatric medical emergencies and trauma cases. We describe the steps involved in implementing Illinois' 3-level facility recognition process to illustrate a model that other states might use to provide appropriate pediatric care and comply with new Health Resources and Services Administration performance measures. Data source: Expert opinion and a search of Index Medicus from January 1986 through October 2001 provided the basis for these guidelines. A task force of experts in the field of patient transport provided personal experience and expert opinion. Study selection and data extraction: Several prospective and clinical outcome studies were found. However, much of the published data comes from retrospective reviews and anecdotal reports. Experience and consensus opinion form the basis of much of these guidelines. Results of data synthesis: Each hospital should have a formalized plan for intra- and interhospital transport that addresses a) pretransport coordination and communication; b) transport personnel; c) transport equipment; d) monitoring during transport; and e) documentation. The transport plan should be developed by a multidisciplinary team and should be evaluated and refined regularly using a standard quality improvement process. Conclusion: The transport of critically ill patients carries inherent risks. These guidelines promote measures to ensure safe patient transport. Although both intra- and interhospital transport must comply with regulations, we believe that patient safety is enhanced during transport by establishing an organized, efficient process supported by appropriate equipment and personnel.
