Emergency Preparedness for CSHCN Using Minnesota EMSC Information System (MEMSCIS)

Project Overview
The goals of this project are to (1) improve emergency and disaster preparedness for children with special healthcare needs (CSHCN) by educating families, primary care providers, pediatric subspecialists, staff nurses, and discharge coordinators at pediatric hospitals about the importance of establishing an emergency medical plan and completing an Emergency Information Form; (2) identify barriers to maintaining emergency preparedness for CSHCN in the community (3) engaging stakeholders in hospital discharge planning in order to improve transitions; (4) enroll hospitalized special needs children in the MEMSCIS Web-Based EIF Database System; and (5) develop an electronic hospital discharge tool that populates an encounter in MEMSCIS. The project will also conduct a mass enrollment of CSHCN as they leave pediatric hospitals and develop a dialogue with the hospitals to facilitate electronic data interchange with MEMSCIS.
